Medication errors are among the most common preventable harms in healthcare. A consistent routine protects every patient. In short: review; perform; check, then complete the remaining steps below.
Step by step
Review the eMAR for new orders, allergies, holds and parameters before you start.
Perform hand hygiene and prepare medications for one patient at a time. Never pre-pour for multiple patients.
Check the medication label against the eMAR three times: when pulling, when preparing and at the bedside.
Identify the patient with two identifiers and check any required parameters, such as apical pulse before digoxin.
Explain each medication and stay until the patient has taken it.
Document immediately after administration, including refusals and PRN effectiveness.
Report any error, refusal or adverse reaction to the RN or provider per policy.
